Compensation
$19 – $21/hr
The Case/Care Manager provides individualized case and care management for high-risk individuals with psychiatric or medical conditions, coordinating services, developing care plans, advocating for community resources, maintaining records, and supporting clients’ independence and integration into the community. The role is full-time, Monday through Friday from 8:00 a.m. to 4:30 p.m., may require use of a personal vehicle, and pays $19–$21.
