Description
The Clinical Activities Specialist (SAC) serves as a clinical leader within the psychosocial component team, supporting practitioners with complex, high-risk, and urgent cases; promoting clinical decision-making, best practices, service pathways, and continuity of care; facilitating clinical meetings and knowledge transfer; monitoring service quality; and supporting interprofessional collaboration. The role requires a bachelor's or master's degree in social work, membership in the Ordre des travailleurs sociaux et thérapeutes conjugaux et familiaux du Québec, and at least two years of recent clinical social work experience. The work model is hybrid, combining home-based interventions with clinical-administrative work, team meetings, and activities at a CLSC or remotely.
