One Health Ohio is hiring a Care Coordinator to support medical care delivery by serving as a communication hub for patients, nurses, doctors, and family members. The role handles patient calls and messages, manages faxes and paperwork, tracks monitors and prescriptions, updates shot records, scans laboratory results into the provider system, assists with referrals and electronic health records, and supports other departments. A high school diploma or equivalent is required, while medical assisting certification and healthcare experience are preferred. The position may work at any OHO locations deemed necessary by the organization.
