Prosano Health Solutions, in partnership with Blue Cross Blue Shield of Arizona, is hiring a Clinical Care Guide (RN) to support its Advanced Primary Care model in Deer Valley, Arizona. The role focuses on in-person and virtual patient support, specialist and diagnostic referral navigation, follow-up, chronic condition support, care team coordination, quality programs, and community relationship building. It requires an active Arizona RN license, an associate degree in Nursing, BLS certification, EMR experience, strong communication and customer service skills, and an Arizona fingerprint clearance card; a BSN and CCM are preferred. The position is 100% on-site at the care center and may involve extended hours and movement between clinics.
