Description
The Case Manager provides comprehensive behavioral health and substance-use case management, including crisis intervention, discharge planning, individual and group work, screening, referrals, coordination with physical health providers, placement support, field services, documentation, and client advocacy. The role requires a master’s degree in a behavioral science, a valid current BBS registration or license such as MFT, LCSW, or LPCC, a California driver’s license, current auto insurance, cultural competency, and shift coverage; mental health or substance-use experience, bilingual ability, and Microsoft Office or EHR knowledge are preferred.
