Description
The Clinical Intake Specialist is part of Vynca’s ECM care management team and conducts outreach and 8–12 daily intake visits with newly referred clients. The role assesses medical and psychosocial needs, reviews charts, documents care plans and goals, communicates client preferences to the care team, and supports complex-care clients using evidence-based practices. The position requires an active California LVN license or a related bachelor’s degree or recognized medical assistant certificate, at least two years of intake or admissions experience, and experience with complex-care populations and health record systems. It is available to applicants in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, and Washington, with remote and in-person work and a Monday–Friday schedule.
