Vynca is seeking an experienced Clinical Lead Care Manager (CLCM) to manage client cases, coordinate healthcare benefits, and educate members on accessing care. This hybrid role involves assessing member needs across various health aspects, developing care plans, connecting clients to necessary services, advocating for patients, utilizing evidence-based practices like Motivational Interviewing, conducting outreach, evaluating progress, providing mental health promotion, arranging transportation, completing documentation, maintaining EMR records, and performing monthly reporting. The ideal candidate will have over two years of experience in care management, possess a bilingual Spanish/English LVN license, and demonstrate strong communication and interpersonal skills.
