Vynca is hiring a Clinical Lead Care Manager in Sacramento County, California, to serve as a client’s primary care coordinator. The role manages care plans, health-care benefits, referrals, outreach, documentation, reporting, and coordination with medical, social-service, and family-caregiver providers. It is a hybrid position requiring travel throughout the Sacramento County area up to five days per week, with candidates required to reside within 25 miles of the assigned territory. The position requires an active California Licensed Vocational Nurse license, at least two years of relevant care-management or community-health experience, bilingual Spanish-English ability, and a valid driver’s license and clean driving record.
