Description
Vynca is hiring a Clinical Lead Care Manager to serve clients in the San Diego area. The role reports to the ECM Clinical Manager, manages client cases and care plans, coordinates healthcare benefits and community services, advocates with providers, conducts outreach, documents progress in EMR and CRM systems, and completes compliance reporting. It is a hybrid position requiring travel throughout the San Diego area up to five days per week, with candidates required to reside within 25 miles of the assigned territory. The role requires a California LPCC, LCSW, or LMFT license, at least one year of relevant care-management experience, and availability Monday through Friday from 8:30 a.m. to 5:00 p.m., with flexibility for evenings and weekends.
