Vynca is hiring a Clinical Lead Care Manager to serve as a client’s primary care coordinator, manage care plans, connect members with health and social services, advocate with providers, and monitor progress. The role is hybrid, requiring field and remote work throughout the Sacramento County area up to five days per week, with candidates residing within 25 miles of the assigned territory. It requires a California LPCC, LCSW, or LMFT license, at least one year of relevant care-management experience, and bilingual English/Spanish ability is preferred. A $2,500 sign-on bonus is available for offers signed by September 30, 2026.
