Vynca is hiring a Clinical Lead Care Manager to serve clients in Sacramento County, California. Reporting to the ECM Clinical Manager, the role manages client cases, coordinates healthcare benefits, develops care plans, connects members with community services, advocates with healthcare providers, and maintains documentation in EMR and CRM systems. The position is hybrid, requiring travel throughout the Sacramento County area up to five days per week and residence within 25 miles of the assigned territory. A California clinical license, at least one year of relevant care-management experience, communication and computer skills, and a valid driver's license are required; bilingual English/Spanish ability is preferred. A $2,500 sign-on bonus is available for offers signed by September 30, 2026.
