Description
The Clinical Nurse will join the Geriatric Outreach Service in Southern Adelaide to provide person-centred, multidisciplinary care to older people in residential aged care homes. Responsibilities include clinical assessment, care coordination, education, support, relationship building, and helping prevent avoidable hospital presentations. The role requires an experienced Registered Nurse with at least three years post-registration experience, complex-need assessment and management experience, and strong communication and clinical judgement. It is available full time or part time, pays $90,913 to $114,917 per year pro rata, and applications closed on 11 October 2026.
