VIVA HEALTH is hiring a Clinical Pharmacist, Coverage Determination in Birmingham, Alabama. The role reviews prior authorizations, formulary exceptions, and other coverage determinations for Medicare and commercial employer groups, researches individual cases, maintains coverage criteria and decision trees, and ensures compliance with CMS and other regulatory requirements. Candidates must be registered or doctor of pharmacy, licensed in Alabama, and have strong communication and computer skills; five years of relevant experience is preferred.
