Description
Spring Health is hiring a Community Care Advocate to work on-site at two locations in the Sacramento, California area, Monday through Friday from 10:00 a.m. to 6:30 p.m. PT. The role connects employees to mental health, financial, legal, housing, food, transportation, and other community resources; provides referrals and follow-up support; coordinates with internal care teams and customer contacts; documents referral outcomes; and promotes the service offering. The position requires a bachelor’s degree in a related field, Spanish and English proficiency, experience working with diverse populations in social work, and strong communication and technology skills. The target salary is $28–$34 per hour.
