Adobe Population Health is hiring a Community Care Coordinator for a per diem, as-needed role providing field-based care coordination, case management, client assessments, resource referrals, health education, home visits, and community outreach. The position serves clients in Eagar, Apache County, and surrounding areas, requires travel and driving, and combines office-based and fieldwork. Candidates need four to six years of related experience or equivalent education and experience, an associate’s degree or active LPN/RN, EMT, or Paramedic certification, current BLS and TB testing, a clear driver’s license, reliable transportation, and strong communication and problem-solving skills; medical experience and bilingual Spanish/English ability are preferred. PRN employees are paid a premium hourly rate and are not eligible for benefits.
