St. Leonard’s Community Services is hiring a Community Case Worker for its Reintegration Program on an 18-month contract. The role provides assessment, case management, day-to-day support, discharge planning, risk and clinical suitability assessment, partnership development, and community-based support for individuals with criminal justice, mental health, and addiction-related needs. The position works Monday through Friday from 8 a.m. to 4 p.m., is based in the community, and requires travel between residential facilities and other community locations. Compensation is $22.95–$24.82, and the role requires post-secondary education, 1–3 years of relevant social-services experience, First Aid/CPR with AED, a valid G-class driver’s license, a reliable vehicle, and a vulnerable-sector police check.
