IMPaCT Care is hiring Community Health Program Coordinators to support Community Health Worker teams across multiple sites. The role coordinates program activities, manages data collection and analysis, maintains documentation and compliance, supports Program Managers, schedules training and community events, and engages with community members and partners. The position is based in West Los Angeles, California, requires 3–5 years of relevant experience, a valid driver's license, car insurance, and the ability to travel across the Atlanta area, with a bachelor's degree preferred.
