Greater Flint Health Coalition is hiring a full-time Community Health Worker for its Genesee Community Health Access Program in Flint, Michigan. The role serves as a liaison between residents and health and social service systems, providing culturally competent outreach, health education, care coordination, case management, system navigation, coaching, social support, and advocacy. The position works with Medicaid, uninsured, and underinsured residents, may support COVID-19 response activities, and requires home visits in Flint and Genesee County. It reports to the Senior Program Director and offers an hourly wage, paid time off, holidays, medical, vision, dental, and other benefits.
