The role builds trusting relationships with disparate population groups, assesses and refers patients, provides culturally relevant health education and screening, coordinates care, documents interventions, and advocates for community populations. It requires a high school diploma or GED, one year of community-related experience, Basic Life Support upon hire, and a valid driver's license; Spanish communication is preferred.
