Description
The Community Health Worker - SUD engages and supports patients in a substance use disorder program through culturally responsive, person-centered care. The role conducts internal and community outreach, identifies potential participants, connects patients with medical, dental, behavioral, and community resources, coordinates care and follow-up, supports current patients in recovery, and may handle intake and scheduling. The position requires strong communication, advocacy, documentation, cultural humility, and teamwork skills, with CSW certification or related training preferred; the pay rate is $24 per hour.
