Description
McGregor PACE is seeking Community Resource Liaisons to serve participants in its Cuyahoga, Lorain, and Summit County PACE programs. The role coordinates with community agencies, healthcare providers, and social service organizations; supports participants with housing and social services; identifies care and communication barriers; schedules community-based services; maintains confidential participant records; and reports to the social worker as part of an interdisciplinary team. Previous community outreach or social services experience is highly desirable, along with communication, local-resource knowledge, teamwork, and Microsoft Office skills.
