Description
The Assertive Community Treatment Social Worker provides comprehensive, person-centered, team-based Assertive Community Treatment services to people with severe and persistent mental illness and co-occurring disorders. Responsibilities include case management, symptom management, supportive psychotherapy, psychosocial rehabilitation, vocational supports, treatment planning, documentation, benefits administration, court advocacy, and collaboration with multidisciplinary teams, families, and community agencies. The role requires a master's degree in social work or a related field, 1,500 hours in a CSP or 3,000 hours working with persons with severe and persistent mental illness, a Wisconsin LCSW or LPC license or ability to obtain one within three years, and a current unrestricted driver's license and auto insurance. The work involves client-home visits, travel, weather exposure, and potential contact with unpredictable or aggressive persons.
