Harbor Health Services is hiring a full-time Complex Care Nurse (RN) for its Harbor Community Health Center-Plymouth. The role supports population health and complex-care services for high-risk patients, including care coordination, assessments, medication management, patient education, health coaching, triage, transitions of care, and documentation in the EMR. The position requires an RN license, current Massachusetts registration, BLS certification, and at least two years of community health nursing experience; a BSN and medical case-management experience are preferred. It offers a $40.00 hourly starting salary, a $5,000 sign-on bonus, and health, dental, vision, life, and disability insurance.
