Description
Greenbrook Medical is hiring a Complex Case Manager to provide longitudinal clinical management for its highest-risk and most medically complex patients. The role develops individualized care plans, identifies deterioration and care gaps, coordinates with physicians, specialists, pharmacists, community partners, and acute-care teams, and tracks action items and outcomes. It requires an active RN or LPN license, at least three years of relevant experience, managed-care or value-based primary-care experience, strong clinical judgment, care-coordination experience, and Excel skills. The position is based in the Greater Tampa Bay Area with daily travel between clinics and care settings, offers a $100,000 base salary plus performance bonus and health, dental, and vision insurance, and includes an application and interview process.
