Description
The Consumer Services Coordinator provides case coordination and advocacy services to persons with developmental disabilities and their families in the Foothill Mountain Adult service area. The role includes Individual Program Plan development, progress monitoring, advocacy, crisis intervention, assessments, placement coordination, facility audits, documentation, and travel to consumers and service providers. It requires a bachelor’s degree plus one year of relevant experience or a master’s degree, offers a $26.3130–$40.8201 hourly range, and includes a hybrid work arrangement with office, remote, and field work.
