Description
The Consumer Services Coordinator provides case coordination and advocacy services to persons with developmental disabilities and their families in the West End area, including travel to Alta Loma, Etiwanda, Rancho Cucamonga, Upland, Montclair, Pomona, Ontario, Chino, Chino Hills, and Mt. Baldy. The role coordinates individual program plans, assessments, placements, crisis intervention, documentation, compliance activities, and interagency meetings, and may provide coverage for absent staff and mentor new employees. It requires a bachelor’s degree plus one year of relevant experience or a master’s degree, offers a hybrid work environment, and pays $26.3130–$40.8201 per hour.
