Description
The Service Coordinator provides information, advocacy, and service-arrangement support for individuals with intellectual and developmental disabilities and their families. The role participates in program planning, coordinates services, documents case information, attends interdisciplinary meetings, identifies service barriers, and provides case coverage and emergency on-call support. It is a community-based position requiring home, community, and office visits, frequent telephone and virtual meetings, a valid California driver’s license, reliable transportation, and the ability to work independently. A bachelor’s degree in a related human-services field and at least two years of related experience are required, with a master’s degree in a related subject substituting for experience. The position offers a hybrid remote option and pays $26.86–$37.96 per hour.
