Description
The Continuous Care Licensed Practical Nurse provides bedside clinical observations, prescribed care, physical and emotional support, and crisis support to patients and families under physician orders and RN supervision. The role accepts and processes physician orders, reports significant changes in patient condition or family needs, and requires an accredited practical nursing education, an active New Jersey LPN license, 1–2 years of clinical experience, hospice knowledge, critical thinking skills, and the ability to travel within New Jersey.
