Description
Jukebox Health is recruiting an Occupational Therapist to conduct in-home assessments and functional evaluations for older adults and individuals with disabilities, provide personalized home-modification recommendations, educate clients and caregivers on fall prevention and safe mobility, participate in collaborative care planning, and maintain electronic documentation. The role is contracted with at least five hours of weekly availability, pays $120 per completed evaluation, and requires an accredited occupational therapy degree, an active unencumbered license, and at least two years of clinical experience.
