Description
Jukebox Health is recruiting a contracted Occupational Therapist to conduct in-home assessments and functional evaluations for older adults and individuals with disabilities. The role focuses on evidence-based home modification recommendations, fall prevention and safe mobility education, collaborative care planning, clinical coordination, and electronic documentation. It requires an accredited Occupational Therapy degree, an active unencumbered OT license, at least two years of clinical experience, reliable transportation, a valid driver's license, and comfort with technology and independent work. The position offers $120 per completed evaluation with a minimum availability of 10 hours per week and an anticipated start date of October 1, 2026.
