The Patient Navigator assists patients through the health care system, advocates for them, coordinates care across hospital admissions, discharges, and emergency department visits, supports disease self-management, facilitates education and medication management, and contributes to quality improvement, data collection, clinical audits, and program evaluation. The role requires a high school diploma, a bachelor's degree and five years of job-related experience, and an LPN or RN New York City license is preferred; bilingual ability is preferred. The stated salary is $56,824.59.
