Description
The Coordinator of Person-Centered Services will provide case coordination and advocacy for individuals with disabilities employed and supported by OBI, including person-centered planning, DDA and DORS funding and authorization work, transportation and direct-care coordination, documentation review, regulatory compliance, family and stakeholder communication, and crisis intervention. The role requires at least one year of experience working with individuals with disabilities or providing case coordination/case management, relevant workforce-development experience, a high school diploma or equivalent, a valid driver’s license, and the ability to perform physical duties such as lifting and transferring participants.
