Description
Cypress Resource Network is hiring a Care Navigator to serve as the primary intake contact for referrals and inquiries across its senior-care pathways. The role coordinates referrals and admissions, triages care pathways, verifies clinical and payer information, supports authorizations and transitions, maintains referral records, and communicates with families, payers, and clinical teams. A high school diploma or equivalent is required; healthcare administration, business, social services, nursing, therapy, or a related degree is preferred, along with at least two years of relevant healthcare intake or care-coordination experience. Full-time medical, dental, and vision insurance are offered.
