Description
The Permanent Supportive Housing Case Manager delivers person-centered case management for 15–20 clients, including home visits, goal-focused support, crisis intervention, self-sufficiency training, referral coordination, and communication with landlords, caregivers, and external agencies. The role also maintains confidential client records and requires a bachelor’s degree in a social service field; one year of case management experience and experience working with homeless, substance-use, or mental-health clients are preferred.
