Description
The Day Services Case Manager provides therapeutic and recreational services, assists in designing developmental skills training programs, and supports individuals to achieve their maximum potential. This role involves developing and monitoring therapeutic service plans, assessing behaviors, documenting progress, and fostering community integration. The ideal candidate will have strong interpersonal and communication skills, basic computer proficiency, and a valid driver's license with a car for client transportation. A preference for candidates who understand and serve culturally diverse populations is strongly encouraged.
