Description
The State of Connecticut is hiring five full-time Developmental Services Case Managers for its Autism Waiver Services Division in the Department of Social Services. The role provides case management, service planning, coordination of medical, educational, residential, and other services, documentation, benefit eligibility support, provider referrals, interdisciplinary planning, and compliance monitoring for individuals with intellectual disabilities and their families. The position is based in Hartford with a Monday–Friday hybrid schedule and may require coverage of statewide regions. Candidates must meet QIDP certification eligibility and either a bachelor’s degree with two years of relevant experience or a master’s degree with one year of relevant experience.
