Description
The Oak Tree Support Services Individualized Home Services (IHS) Specialist supports adults with disabilities in their homes and communities by maintaining a caseload, coordinating with clients and support teams, documenting visits and cancellations, helping clients build community skills, transporting clients to appointments and errands, and completing required reporting and training. The role requires a valid driver’s license and insurance, a high school diploma or GED, reliable transportation, and the ability to pass a DHS background study.
