Description
The Discharge Planner receives referrals for at-risk patients and families, conducts psychosocial assessments, provides interventions for abuse, violence, neglect, and end-of-life care, and coordinates complex discharge plans with case managers and healthcare teams. The role connects patients to community resources, supports eligibility and funding needs, and handles issues involving guardianship, mental health placement, advance directives, foster care, adoption, and protective services. A CSWE-accredited BSW or MSW, current professional licensure or certification, strong clinical judgment, communication, collaboration, problem-solving, computer literacy, and data analysis skills are required or preferred.
