Description
The Care Coordinator supports the Case Management and ambulatory care management teams by coordinating care for commercial outpatient populations, developing care plans, arranging post-acute and transportation services, supporting discharge planning and Transitions of Care, collecting and documenting patient data, and collaborating with physicians, social workers, pharmacy, behavioral health, and facility staff. The role requires a high school education, three years of clinical experience, computer data-entry experience, typing of 40–55 WPM, organizational and customer-service skills, and Fire Life Safety Training in Los Angeles City. The hourly rate is $29.00–$45.20.
