The role includes client outreach, care-plan creation, clinical sign-offs, interdisciplinary team meetings, crisis guidance, performance coaching, and collaboration with broader care teams. It requires an active, unrestricted California LCSW or LMFT license, 2 years of people-management experience, 2–3 years of care-management experience, and 1–2 years of post-acute experience. The position offers $90,000–$115,000 annually, with a higher California range of $100,000–$125,000, and includes medical, dental, and vision insurance for full-time employees.
