Description
The Case Manager provides comprehensive case management for clients, including crisis intervention, discharge planning, group and individual work, mental health and substance use disorder counseling, screening, symptom management, referrals, and coordination with physical health providers. The role also supports educational and school placements, IEP and wraparound meetings, family education, documentation, confidentiality, and field-based services, with part-time on-call coverage. Qualifications include a master's degree or a bachelor's degree with four years of social-services experience, a valid California driver's license, current auto insurance, cultural competency, and preferred experience in mental health or substance use services, bilingual ability, and Microsoft Office and EHR software knowledge.
