Description
The Case Planner supports the safety, permanency, and well-being of children and youth in foster care by conducting face-to-face contacts, assessing safety and risk, developing and monitoring service plans, coordinating with families and service providers, intervening in crises, supporting permanency planning, and attending court and interdisciplinary meetings. The role is full time and non-exempt, reports to a Case Planning Supervisor, and requires a bachelor’s degree, background clearances, communication and computer skills, a valid New York State driver’s license, and willingness to travel between agency sites.
