Description
The Case Planner supports the safety, permanency, and well-being of children and youth in foster care by conducting face-to-face contacts, assessing safety and risk, developing and monitoring service plans, coordinating referrals and treatment, supporting families through court and permanency processes, intervening in crises, and attending interdisciplinary meetings. The role requires a bachelor’s degree, BSW or MSW preferred, background clearances, communication and computer skills, a valid New York State driver’s license, and willingness to work extended or irregular hours and travel between agency sites.
