Form Health is hiring an Endocrinologist specializing in obesity medicine for a fully remote, W2 part-time position in the United States. The role provides telemedicine patient care for obesity and diabetes, manages complex comorbidities, supports insulin and CGM therapy, collaborates with advanced practice providers and dietitians, and contributes to quality improvement and clinical mentoring. Candidates must have an MD or DO degree, ABIM Endocrinology certification, a valid unrestricted US medical license, Medicare opt-in, and at least two years of relevant clinical experience or an accredited fellowship. The position requires a minimum of 20 hours per week, including 16 hours of clinical patient-facing time and 4 hours of administrative time, with a base annual salary of $94,500 to $112,500 USD.
