Form Health is hiring an Endocrinologist specializing in obesity medicine for a fully remote, US-based W-2 position. The role provides telemedicine patient care focused on obesity treatment and comprehensive diabetes management, including insulin titration, CGM and insulin pump therapy, complex co-morbidity care, medical management, and collaboration with advanced practice providers and dietitians. It requires an MD or DO, ABIM Endocrinology certification, a valid US medical license, Medicare opt-in, and at least two years of relevant clinical experience or an accredited fellowship. The position offers 32, 36, or 40 hours per week, with 80% clinical and 20% administrative time, and a base annual salary of $210,000–$250,000 USD plus health, retirement, leave, and other benefits.
