Description
The Case Manager provides direct service and support to individuals and families, conducts interviews and needs assessments, coordinates referrals and appointments, initiates emergency intake, maintains client records, and helps clients develop sustainability plans. The role also involves building relationships with landlords and apartment complexes, participating in training and staff meetings, and entering data into an internal database. An associate degree in Human Services or bachelor's degree in Social Work is a plus, and starting pay is up to $40,000.
