Evening Manager l CASAC, LMSW at Arms Acres

The employer is hiring two Adult Program Clinical Case Managers to provide clinical case management, including assessments, individual, group, and family treatment, treatment-plan coordination, referrals, discharge planning, and intervention with client support systems. The role also oversees caseloads and scheduling, coordinates patient transfers and census, supports treatment planning, addresses conflicts, conducts wing meetings, reviews program services, and participates in performance improvement initiatives. Candidates must hold New York State CASAC and/or Licensed Master of Social Work credentials. The schedule is Sunday through Thursday and Tuesday through Saturday from 2:00 p.m. to 10:30 p.m., with a salary of $20–$25.
