Description
The Behavioral Health Case Manager provides comprehensive assessment, family action planning, crisis prevention, wrap-around support, home, community, and school visits, coordination of psychiatric and other services, continuity of care, discharge planning, advocacy, and 24-hour crisis coverage for children and youth with complex needs and their families. The role requires a bachelor’s degree in a human services field, one to two years of behavioral health experience, and a driver’s license and vehicle; experience working with children and adolescents is preferred.
