Description
The Family Practice Provider role delivers full-scope outpatient primary care across diagnosis, treatment, care coordination, preventive care, patient education, and electronic health record maintenance. The position requires an osteopathic or medical doctor, completed primary-care residency, board certification, Illinois licensure, DEA and controlled-substance registration, and current CME; two years of outpatient primary-care experience is preferred.
