Description
Rising Ground is hiring a full-time Case Planner to provide direct case-planning services for children in foster care, connect families with services, maintain mandated contacts, develop service plans, document reports, conduct home visits and collateral contacts, attend Family Court hearings, and participate in service-plan reviews and Family Team Conferences. The role is based in Brooklyn or Bronx, requires in-person work at least three days per week, and offers medical, dental, and vision insurance.
